Borrower Registration, PIN and Online Access Request

The following forms are for use by staff currently employed in WA Health mental health services, for work-related purposes. If you are studying at university, please use the Library staff and resources at your university to assist you with your non-work related requests.

Fair dealing provisions in most instances restrict us to providing you with one article from a journal issue or one chapter from a book.

Ask us for information on any work-related topic. Due to time constraints the Library team will provide you with Literature Search guidance for you to locate your own list of references for your Literature Search, including use of Endnote and Search techniques. If you wish to request a journal article, book chapter or loan please use Request for loans or copies.

If you would like the library to purchase a book, journal or DVD for the library collection.
If you would like the library to purchase a book, DVD or journal to be stored in your department and paid for from your cost centre. This needs to be authorized by the relevant Cost Centre Manager.
